These are common questions I get every week. In general, pregnancy changes your breasts in several ways and in doing so, changes the way your soft tissue interacts with your breast implants. Pregnancy will physiologically and anatomically change your breasts in the same way with or without implants. However, if you do have implants, they often will not look the same as they did prior to pregnancy.

Do Breast Implants Change During Pregnancy?

The good news: breast implants themselves don’t change. What can change is the surrounding tissue, skin, and breast structure.

During pregnancy, your body undergoes hormonal shifts that can cause:

  • Breast swelling
  • Skin stretching
  • Milk gland enlargement
  • Temporary or permanent changes in nipple sensitivity

These natural effects can impact the appearance of your breasts regardless of if you have or don’t have implants.

Common Post-Pregnancy Breast Changes (With Implants)

Here’s what some women notice after pregnancy and breastfeeding:

1. Breast Sagging (Ptosis)

Weight gain, milk production, and skin stretching can lead to breasts drooping even with implants. The skin may no longer hold the implant firmly in place.

2. Loss of Upper Pole Fullness

The top part of the breast may appear deflated, while the bottom remains fuller. This is especially common after breastfeeding.

3. Volume Changes

Some women experience a loss of natural tissue volume, which can alter the shape of the implant over time. Others feel their breasts look too large post-breastfeeding.

4. Capsular Contracture Risk

Though rare, changes in breast tissue during and after pregnancy could slightly increase the risk of scar tissue tightening around the implant.

Should You Wait Until After Pregnancy for Breast Implants?

This depends entirely on your life plans.

Breast augmentation is safe before, after, or between pregnancies. However, if you’re actively planning a pregnancy within the next 1–2 years,  I would recommend waiting to avoid unpredictable post-pregnancy changes.

When you don’t need to wait:

  • You’re not sure if or when you’ll have children
  • You’ve finished breastfeeding and want to restore your pre-baby look
  • You accept the possibility of needing revision surgery later

What If I Already Have Implants and Become Pregnant?

No need to panic, pregnancy is completely safe with breast implants. They don’t affect your ability to carry a baby or breastfeed (in most cases).

Here’s what you can do:

1. Monitor Your Body

Pay attention to how your breasts change. Note any unusual pain, hardness, or distortion though these are rare.

2. Wear Proper Support

A supportive maternity bra helps reduce strain on the breast tissue and skin, especially with implants.

3. Avoid Drastic Weight Fluctuations

While some weight gain is expected, try to maintain a healthy range. Excessive stretching can worsen sagging.

Can You Breastfeed with Implants?

How pregnancy affects breast implants

In most cases, yes. 

However, your ability to breastfeed depends on several factors:

  • The surgical technique used
  • Nipple sensitivity and anatomy
  • Your body’s natural ability to produce milk

A multicenter retrospective study showed that women with implants placed under the muscle had an 82% probability of breastfeeding successfully versus only 17% who had implants placed under the gland. The type of incision used during your breast implant surgery also has an impact with an incision under the breast being better for breast feeding than an incision around the areola.*

While I prefer to place implants above the muscle for a variety of reasons, if you are concerned about your ability to breastfeed, then an under the muscle approach through an incision under the breast is the best approach.

Do I Need a Breast Revision After Pregnancy?

Not always. Some women are pleased with their post-pregnancy appearance and choose not to pursue revision. However, if you’re unhappy with volume loss, sagging, or shape, you may benefit from:

  • Implant Exchange: Swap your current implants for a different size or type to better fit your post-pregnancy body.
